There are several ways that this phrase may be interpreted. It could be a hendiadys - "teaching with all patience" and thus a further command. It could be providing the means of preaching. It could be providing the means of all five imperatives. It could be jut modifying the last three imperatives: reprove, rebuke and exhort. I have chosen the later, because the phrase is far away from preaching and it doesn't make sense to apply it to "be ready in season and out of season".
