He is still before his enemies (13-14), but hoping in God to respond - to answer to himself, and his enemies (vs. 15-16) Though a matter of interpretation, I see 18-22 as David's final cry for help. ≈He states his reason for his cry: though he is confessing his sin, though he has hoped in God, things aren't getting better, his enemies aren't giving up on harming him (vs. 18-19), and they are continuing to accuse him because David pursues something they don't (vs. 20).
