The central idea of this passage is that God's word is powerful to see within and judge our hearts. I don't really know if this is allowed, but the idea of 12a actually receives three explanation labels in ever-widening arcs. As indicated by my sticky, I feel that 12e is the centre of the explanation -- the word of God "is living and active...discerning the thoughts and intentions of the heart". Vs. 13 expands on this, explaing the universal reach of this judgment. And the comparison in vs. 12b-d also provides another layer of explanation of the initial proposition, "the word of God is living and active". Thanks for your feedback!
